New licence applications
Any person or business operating any of the listed licensable activities will need to apply for a licence, and will need to comply with the new legislation and the conditions set by this legislation.
To apply for a new licence, download the relevant licence application form below, complete and send it to: Environmental Health, Woking Borough Council, Civic Offices, Gloucester Square, Woking GU21 6YL.

Existing licence holders
Prior to the expiry date of a licence, we will send the existing licence holder the relevant applications forms and notification of fees.
If you are the holder of an existing licence, you are advised to familiarise yourself with the new licence conditions.
