Your application should be accompanied by the following:
- a plan of the premises
- the required fee
- an operating schedule
- a copy of the club rules
- a declaration form
You are required by law to give a copy of your application, along with the above documentation, to all responsible authorities in the area in which the premises are situated. This must be done on the same day you submit your application.
