The Homesafe Plus service provides support, assistance and reassurance to patients and their families following a stay in hospital.
Homesafe Plus also aims to reduce pressure on local NHS services by:
- facilitating timely discharge from hospital and minimising excess bed days
- reducing preventable hospital admissions
- helping patients to access support provided by local authorities.
Homesafe Plus tailors existing borough services to provide immediate support after hospital discharge and, over the course of six weeks, links patients to a variety of services available. Working in partnership with health and social care across north west Surrey, Homesafe Plus is jointly provided by Elmbridge, Runnymede, Spelthorne and Woking borough councils.
Before you leave hospital, a hospital member of staff will discuss and recommend the service options available to you and, if it’s appropriate, complete a referral form on your behalf. The completed referral form will then be sent to us.
